Memorial Plaque Application Disclaimer

Each Memorial Plaque is presented to the Next of Kin of a Fallen Hero to honor their sacrifice. Due to limitations, we can only provide one plaque per hero.

Relationship: When they are "Next of Kin" (Listed in Order of Priority)
Spouse	Always
Child (Biological or Adopted)	Always, unless parental rights were terminated via adoption.
Grandchild	When the child/parent who the decedent/grandchild have in common is deceased.
Great Grandchild	When the decedent had none of the above surviving at the time of death.
Parents	When the decedent had none of the above surviving at the time of death.
Sibling	When the decedent had none of the above surviving at the time of death. This applies to full and half, but not step siblings.
Grandparent	When the decedent had none of the above surviving at the time of death.
Other Relatives	If none of the above apply, other relatives may be Next of Kin.

To ensure fairness and accuracy, applications must be submitted with complete and correct information. Incomplete or incorrect submissions will be denied.

Notice: If there is an issue with the information submitted on the plaque application (Next of Kin, Photo, Address, etc.) and we cannot get a hold of you (after 30-days of contact attempts), we will unfortunately, have to deny the request for a Memorial Plaque and your application will be archived. *
